Premium Subscription (Per Provider Per Month)  Premium Subscription:  The McKesson Practice Choice Premium Monthly Subscriptions licenses a provider with a cost-effective, web-based electronic health record (EHR) and practice management (PM) solution that is ONC-ATB Stage 1 certified to enable Meaningful Use submission.  The solution license includes the following components: Integrated PM and EHR Administration, Practice Scheduling with Wait/Bump list management and Real-time/Batch Eligibility, Charting including Chart Summary, Patient History, Vitals, Diagnosis & Procedures, Orders, Results, HealthMaintenance with Alerting, Documentation Library, and Continuity of Care Document Publishing, Inbound Lab Results interface licensing  (Availability of lab interfaces depends on technical compliance and contractual agreement of theintegrating lab suppliers with McKesson Practice Choice and the Customer.  Payment for each connection will be required of the connecting HIE, lab supplier or practice.  The list of currently available lab suppliers can be found at: www.mckessonpracticechoice.com/labinterfaces) , Clinical Encounter Summary and Progress Notes leveraging Smart Note technology to push and pull from the Patient Chart, Billing with Claims, Payment and Statements including an embedded EDI/ERA service with ERA auto-posting, Electronic Prescribing with Medication/Allergies including drug-drug, drug-allergy checking, Internal Communications for intra-practice messaging and tasking linked to an online Patient Portal for patient interaction, Native Reporting including Stage 1 Meaningful Use measures, certain ClinicalQuality Measure reports, Syndromic Reporting, and other financial reports.	$399 per month

Implementation and Training Costs:  (Required)
Premium Implementation per 1st Provider:  McKesson Practice Choice Premium Implementation for the First Provider delivers up to 28 hours of remote project management and/or remote training services for the purpose of aiding the practice and the first provider in adoption of the solution.  In addition, this Implementation package includes up to ninety days ofPremier Support (as described in the then-current McKesson Practice Choice Customer Care Terms found at www.mckessonpracticechoice.com/customercareterms) beginning upon the Commencement Date. The practice will need to engage its own resources to be successful in the implementation as described by the then current McKesson Practice Choice Implementation Guide found at  www.mckessonpracticechoice.com/implementationguide as such terms may change over time.	$4,700 for first provider, $1,450 for additional provider.

Implementation of the submission may be subject to a to-be-determined implementation time.	$70 per month
Patient Education Subscription:   The McKesson Practice Choice Patient Education Monthly Subscription licenses a provider with patient instruction content on over 5,000 health topics from McKesson’s Clinical Patient Education.	$45 per month
